Cedar Point Health is growing and seeking a passionate Nurse to join our team. The Annual Wellness Visit Nurse (AWV) is responsible conducting patient Annual Wellness Visits by working in collaboration with the primary care provider and all members of the health care team.

Responsibilities:

  • Review clinic schedule and identifies patients appropriate and due for Medicare AWV.
  • Work collaboratively with clinical teams to accomplish AWV in an efficient manner for patient and team.
  • Follow AWV protocols and standing orders including completion of preventive services that may result from AWV.
  • Develop and shares knowledge of AWV eligibility and documentation requirements.
  • Develop knowledge of Medicare preventive services and clinical guideline recommendations for those services.
  • Appropriately documents within the electronic record to satisfy closed quality gaps.
  • May assist with other clinical duties such as prior authorizations, patient messages, prescription refills.
  • Utilize appropriate protocols and standing orders in delivering patient care.
  • Participate in regular team meetings, huddles, staff meetings and quality improvement projects to improve patient care.
  • Consult with the clinical staff and ancillary department staff to eliminate barriers to the efficient delivery of care. Identify service delivery problems and potential for effective patient management intervention.
  • Ability to work independently and exercise clinical judgment in interactions with Physicians, payers, patients and their families.
  • Document all communication with patient in electronic medical record.
  • Maintains strict confidentiality, adheres to HIPAA guidelines and regulations.
  • Performs other duties as assigned.

Job requirements

  • Current State LPN or RN License
  • Knowledge of medical model specific to Annual Wellness Exams
  • Knowledge of patient assessment techniques including taking medical histories, performing physicals, evaluating health status including state of wellness, and compliance with care recommendations.
  • Knowledge of how to use medical equipment and instruments to administer patient care.
  • Knowledge of medical records and documentation in confidential manner.
  • Ability to make responsible decisions within scope of practice.
  • Ability to collaborate effectively with physicians.
  • Ability to maintain quality control standards.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to interpret, adapt, and apply guidelines and procedures.
  • Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff, and the public.
  • Position is required in-person.

Education: Successful completion of approved LPN or RN program.

Experience: 2-4 years of experience performing Annual Wellness Visits in a clinical setting.

Mental and Physical Requirements: Varied activities including standing, walking, reaching, bending, and lifting. This position requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Must be able to use a variety of office equipment. May require working under stressful conditions.

Conditions: Normal office and exam room environment. The position requires the ability to work under pressure and with a diverse population, including staff, physicians, clients, patients, insurance companies, labs, hospitals, and other members of the public on a regular basis. The position may cause exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinical environment.
