In collaboration with the patient’s healthcare team, a care manager guide patients through their care plans by providing assistance, education, and support.

Responsibilities:

  • Collaborate in overseeing the practice’s chronic care management program, including Medicare’s CC requirements.
  • Assist with and facilitate the transition of care from inpatient settings such as hospital, rehabilitation facilities and skilled nursing facilities to home.
  • Develop a comprehensive, collaborative care plan, based on provider treatment plan, evidence-based chronic care guidelines, and patient/family goals for patients in the chronic disease registries, recent discharges, and those in care transition to promote adherence to physician/provider recommendations and instructions.
  • Provide individual patient/family education and self-management support that is appropriate based on language, cognitive abilities, literacy level, learning style, cultural norms, patient preference, readiness for change and resources available.
  • Address medication adherence and reconciliation.
  • Provide education to patients regarding health conditions.
  • Communicate changes in patient’s status appropriately with the care team.
  • Identify barriers when treatment goals are not met, treatment plan is not being followed or important appointments are missed.
  • Identify patients who are overdue for visits, labs and referrals, contact patients and arrange for follow-up services.
  • Help the practice and providers meet the requirements and goals for Meaningful Use, PQRS, Quality and other regulatory programs.

Job requirements

Requirements:

  • Must be experienced in a physician’s office or equivalent combination of training and experience.
  • Must have experience with medical documentation, and EMR’s
  • Must have knowledge of examination, diagnostic and treatment room procedures.
  • Skilled in developing and maintaining clinical quality assurance.
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Ability to use good reasoning and judgment and react calmly in emergency situations.
  • Ability to establish and maintain effective working relationships with patients, medical staff, coworkers and the public.
  • Ability to read, write and communicate effectively orally and in writing.
  • Must be able to multi-task.
  • Must be punctual, meticulous and reliable.
  • Must be able to maintain strict confidentiality.

Education: Medical Assistant Certification, RN, or LPN (Pay will reflect certifications and experience).

Experience: Preferred to have experience in a medical office, nursing home, or inpatient facility. Experience in care management or population health.
