Cedar Point Health is growing and is looking for a Care Manager to join our primary care team in Montrose, Colorado. As part of an integrated, team-based care model, the Care Manager collaborates closely with primary care providers, medical assistants, and other members of the primary care clinic. This role supports patients with high-risk and medically complex conditions by promoting compliance, connecting them with resources, and delivering ongoing education and support. The Care Manager acts ethically and responsibly with medical documentation in the Electronic Medical Record (EMR), contributing to improved patient satisfaction, better health outcomes, reduced risks, and fewer acute care episodes.

Responsibilities

  • Collaborate in overseeing the clinic’s Chronic Care Management (CCM) program, including compliance with Medicare CCM requirements.

  • Assist with and facilitate transitions of care from inpatient and outpatient settings (e.g., hospitals, rehabilitation centers, and skilled nursing facilities) to home.

  • Develop comprehensive, collaborative care plans based on evidence-based guidelines and aligned with the provider’s treatment plan.

  • Provide individualized patient/family education and support for self-management, tailored to the patient’s health conditions and needs.

  • Address, educate, and reconcile quality measures to support value-based care.

  • Communicate relevant changes in a patient’s health status with the care team in a timely and effective manner.

  • Identify and work to address social determinants of health affecting patient outcomes.

  • Build trust with patients to promote engagement and adherence to care plans.

  • Maintain compliance with HIPAA and act ethically in all aspects of care coordination and documentation.

  • Build and maintain relationships with external care providers, community partners, and organizations to support patient needs.

Job requirements

Requirements

  • Experience working in a physician’s office or equivalent combination of training and experience.

  • Proficient with EMRs and accurate, ethical medical documentation practices.

  • Knowledge of examination, diagnostic, and treatment room procedures.

  • Familiarity with medical equipment, instruments, and safety protocols.

  • Strong multitasking skills and attention to detail.

  • Punctual, reliable, and respectful of patient and co-worker confidentiality.

  • Ability to work on your feet for extended periods and occasionally lift up to 20 pounds.

  • Team player with the ability to foster collaborative working relationships across disciplines.

Skills & Abilities

  • Ability to apply policies and procedures consistently and appropriately.

  • Calm, effective responses in urgent or emergency situations.

Education

MA, LPN, or RN preferred.

Experience

Preferred experience in a medical office, nursing home, or inpatient facility. Prior experience in care management or population health is highly favored.

Mental and Physical Requirements

Varied physical activity including standing, walking, reaching, bending, and lifting. Requires full range of motion, including handling patients, manual dexterity, and eye-hand coordination. Comfortable using a range of office and clinical equipment. Must be able to work under occasional stressful conditions.

Working Conditions

Standard office and clinical environment. Must be comfortable working with a diverse population, including patients, providers, insurance representatives, and external agencies. Potential exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, and medical preparations typical of a healthcare setting.
